Chapel Gate Medical Centre is committed to upholding patient confidentiality in accordance with best practice medicine. All patient details and personal health records remain private and the information is only accessible to authorised staff.
The doctors may take phone calls relating to you and your family’s health. However if they are seeing patients when you call, they may need to call you back.
Chapel Gate Medical Centre is a private billing clinic. Payment for the consultation is expected on the day. We can submit your paid account online to Medicare for claiming and refund.

Pensioners and Veterans’ Affairs card holders are Bulk Billed.

Information about our fees is available at reception. Fees vary according to the complexity of service or if a procedure is performed.

Please ensure you bring your Medicare card, Health Care Card or Pension Card to your appointment.

New patients are welcome at our Clinic.

All new patients are required to complete our “New Patient Form”. If you are new to our clinic please allow 15min before your appointment time to arrive and fill in the Patient Registration Form and present it to our reception staff.

Consultations at the Chapel Gate Medical Centre are by appointment. A standard appointment is 15 minutes. Every effort is made to provide an appointment with the Doctor of your choice; however, for urgent matters this may not be possible. We try to keep as closely as possible to the appointed times, however if we are unable to do this, staff will inform you of any expected delays.

If you feel you need to see the Doctor for a complex medical issue, or have more than one problem, please inform reception when booking your appointment so that a longer appointment can be made.

Please ensure your telephone number and address is correct and up to date.

We try to keep waiting times as short as possible, it will not always be possible for you to be seen at exactly the appointed time. We ask you to be tolerant if this occurs. Our reception staff are able to advise of expected waiting times upon arrival.
We operate a computer based medical record system. Should any of your personal details change (Address, Phone number, Medicare Number etc.), please inform our reception staff to have your records amended.
If an interpreter is required, the Translating and Interpreting Service (TIS National) can be contacted on 131 450.

Chapel Gate Medical Centre prides itself on the level of care we provide to our patients.

To maintain the best level of care and for the continuity of medical management you are required to book a brief appointment with your doctor for all scripts, referrals and test results.

In exceptional circumstances repeat prescriptions may be given without consultation for a fee.

Our practice is committed to preventative health care. We may send you a reminder notice from time to time offering preventative health services appropriate to your care. If you do not wish to be part of this system please let us know.

After Hours

Chapel Gate Medical Centre provides our own after hour’s phone triage service, using RACGP fellowed GPs working within the company, Medical and Aged Care Group.

Parking is available at the rear of the clinic, accessible via the lane next to the building or across the road near Windsor railway station or at Windsor Car Park (off Chapel Street at the rear of “Brian’s Paint Spot”).

We are conveniently located only a short walking distance from Windsor Railway Station.
We encourage patients to come to the clinic for a consultation so their Doctor can deliver optimal care with full access to clinical facilities and equipment.

Regular patients who are unable to attend the Parkdale Medical Surgery will be referred to their usual doctor who will assess the home visit request and make appropriate arrangements for the patient.
